Pursuant to LBR 5005-4(c), the Balance of Schedules and/or Chapter 13 Plan (CSD 1099) must provide the debtor(s)' original signature(s) in a scanned format. ( "/s/ typed name of debtor" will not be accepted.) However, the attorney signature may be in "/s/ attorney name" format .

The Balance of Schedules and/or Chapter 13 Plan docket entry satisfies filing deadlines of required documents which were not filed at the time the Bankruptcy Case was filed. If the Chapter 13 Plan and/or the Statements of Current Monthly Income Forms 122A-1, 122A-1Supp,  122A-2, 122B,  122C-1, or 122C-2 are indicated on the Balance of Schedules, they must be filed as separate document entries after the Balance of Schedules is filed.

If documents filed via the Balance of Schedules include additional creditors not originally listed on the creditor matrix at the time the Bankruptcy case was filed, the Notice to Creditors (CSD 1101) is required to be filed under its own document event and referred (linked) to the appropriate Balance of Schedules.

  1. Select Bankruptcy menu.
  2. Select Miscellaneous Documents.
  3. Ensure correct case name and number match the document you are filing.
  4. Select party filer.
    • Party filer not listed, Add/Create New Party.
  5. Select Document Event: Balance of Schedules and/or Chapter 13 Plan (CSD 1099)(fee).
  6. Browse, verify and attach the CSD 1099 (PDF file).
    • Attach supportive documents, such as the Balance of Schedules, if applicable.
  7. Select yes or no for Are Creditors being added that were not on the original list of creditors when the case was filed?
    • Yes ($32.00 fee)
    • No
  8. Click on Submit.
  9. Select one or more schedules/statements being filed:
    • Schedules A/B - J
    • Statement of Financial Affairs
    • Summary of Assets and Liabilities and Statistical Information
    • Statistical Summary (cases filed prior to 12/1/15)
    • Statement of Current Monthly Income (122A-1, 122B, 122C-1)
    • Statement of Exemption from Presumption of Abuse (122A-1Supp)
    • Means Test Calculation (122A-2)
    • Calculation of Disposable Income (122C-2)
    • Chapter 13 Plan
    • Schedule A/B - Property
    • Schedule C Property Claims as Exempt
    • Schedule D Claims Secured by Property
    • Schedule E/F Unsecured Claims
    • Schedule G Executory Contracts and Unexpired Leases
    • Schedule H - Codebtors
    • Schedule I - Income
    • Schedule J - Expenses
    • Schedule J-2 - Expenses for Separate household of Debtor 2
  10. Enter Summary of Schedules and Statistical Summary information.
    • Enter the dollar amount as shown, or leave blank if data not available.
    • Bypass this screen if not filing Summary of Schedules and/or Statistical Summary information.
  11. Modify text to add pertinent information, such as Amended, if applicable.
  12. Verify final docket text before submitting onto case docket.
  13. Print the Notice of Electronic Filing. It's your receipt.
  14. Continue with filing the Chapter 13 Plan,; Statement of Current Monthly Income Form 122A-1, 122B, or 122C-1; Statement of Exemption from Presumption of Abuse Form 122A-1Supp;  Means Test Calculation 122A-2;  Calculation of Disposable Income 122C-2; or Notice to Creditors (CSD 1101), if applicable.

Quick Note

DO NOT include or file the Certificate of Credit Counseling via the Balance of Schedules. It should be filed under its own document event.
