IMPORTANT: Before completing and filing this Form 122C-1 form, read the accompanying instructions to ensure accuracy of the information entered on the form
 

  1. Select Bankruptcy menu.
  2. Select Miscellaneous Documents.
  3. Ensure correct case name and number match the document you are filing.
  4. Select party filer.
    • Party filer not listed, Add/Create New Party.
  5. Select Document Event: Statement of Monthly Income(Ch13) (122C-1).
  6. Browse, verify and attach the CSD 122C (PDF file).
    • Attach supportive documents, if applicable.
  7. Enter Current Income of Individual Debtor(s) from Form 122C-1, Line 14.
  8. Select yes or no for Is Disposable Income Determined? Form 122C-1, line 17a or 17b.
    Select Yes if determined and File Form 122C-2 separately
  9. Select 3 years or 5 years for Is Commitment Period for 3 or 5 Years?
  10. Modify text to add pertinent information, such as Amended, if applicable. 
  11. Verify final docket text before submitting onto case docket.
  12. Print the Notice of Electronic Filing. It's your receipt.

Quick Note

 

If this Statement of Current Monthly Income is part of the Balance of Schedules, file the Balance of Schedules (CSD 1099) and Statement of Current Monthly Income (122C-1) as separate documents.
