Pursuant to LBR 5005-4(c), the Amendment (CSD 1100) must provide the debtor(s)' original signature(s) in a scanned format. ( "/s/ typed name of debtor" will not be accepted.) However, the attorney signature may be in "/s/ attorney name" format .
  
When adding creditor names and addresses to the case via an Amendment, the Notice to Creditors (CSD 1101) must be filed also. When the Amendment is amending other case related items, which are not adding creditor names and addresses, the Notice to Creditors is not required.

If the Notice to Creditors is required, it must be filed under its own document event and referred (linked) to the appropriate Amendment.

  1. Select Bankruptcy menu.
  2. Select Miscellaneous Documents.
  3. Ensure correct case name and number match the document you are filing.
  4. Select party filer.
    • Party filer not listed, Add/Create New Party.
  5. Select Document Event: Amendment (CSD 1100)(fee).
  6. Browse, verify and attach the CSD 1100 (PDF file).
    • Attach supportive documents, such as amended schedules.
  7. Submit.
  8. Select one or more boxes as appropriate (reason for amendment).
  9. Select the appropriate statement which Requires Compliance with LRB 1009:
  10. Submit.
  11. Modify text to add pertinent information and Amended, if applicable.
  12. Verify final docket text before submitting onto case docket.
  13. Print the Notice of Electronic Filing. It's your receipt.
  14. Continue with filing the Notice to Creditors (CSD 1101) ONLY when Amendment is adding creditors.

Quick Note

DO NOT include or file the Statement of Current Monthly Income  (B22A-1or 122A-1),(B22B or 122B),  (B22C-1 or 122C-1);  Statement of Exemption from Presumption of Abuse( B22A-1Supp or 122A-1Supp); Means Test Calculation (B22A-2 or 122A-2); Calculation of Disposable Income (B22C-2 or 122C-2); Certificate of Credit Counseling; nor Chapter 13 Plan  via the Amendment.  They should be filed under their own document event.
