Before filing the proof of claim, remember to
redact
personal information as required by Bankruptcy
Rule 9037.
Filing the proof of claim allows you
to select the creditor from the creditor's list. If the
creditor's name is not listed, Add the Creditor during filing.
The creditor information is used for noticing purposes.
Entering creditor's data in the Name and Address field cannot
exceed five lines per creditor. If multiple creditors are
added, separate each added creditor with a blank line.
- Select
Bankruptcy menu.
- Select
File Claims.
- Ensure
correct case name and number match the document you are filing.
- Search
for the appropriate creditor from the drop down list.
- Is the creditor listed? (Select
the appropriate answer for instructions.)
- Proof
of Claim Information for Screen.
- Amends
Claim # (Bypass - see File
Amended Claims procedure to file an amended claim)
- Filed
by (select appropriate party type filing the claim)
- Enter
amount(s) in the Amount
Claimed, Secured
and/or Priority
sections..
- Note to the U.S. Trustee:
If filing an Administrative claim, select Priority.
- Description
and/or Remarks, if applicable.
- Browse,
verify and attach the Proof of Claim (410 form) (PDF file).
- Attach
supportive documents,
such as pay stub, contract, etc., if applicable.
Note:
For Mortgage claims filed in a Chapter 13 case, attach
the Mortgage Proof of Claim Attachment (410A) as a supportive
document.
- Print
the Notice of Electronic Filing. It's your receipt.
- Make
note of assigned proof of claim number.

The new Bankruptcy Rule 3002.1 requires
that when filing a Mortgage Claim in a Ch 13 case, the
Mortgage Proof of Claim Attachment (410A) must be attached
as a supportive document to the proof of claim.
