Before filing the proof of claim,
remember to redact
personal information as required by Bankruptcy Rule 9037.
Filing the amended proof of claim allows
you to modify information entered on the corresponding original
claim. Access the claims register to
identify the claim number of the original claim to be amended.
If the creditor name and address on the original and amended
claims do not match, see File
New Claim to file the claim.
- Select
Bankruptcy menu.
- Select
File Claims.
- Ensure
case and number match the document you are filing.
- Enter name of creditor or submit.
- Select
the from the drop down list.
- Review claim name and address
in the pop up box. Is this the correct creditor? (Select
the appropriate answer for instructions.)
- Proof
of Claim Information Screen.
- Enter
claim number of claim to be amended in Amends
Claim # box.
- Click on
appropriate creditor name with claim number to be amended.
- Current
claim information will populate claims register.
- Modify
and/or add appropriate information for the amended claim.
- Browse,
verify and attach the Proof of Claim (B10 form) (PDF file).
- Attach
supportive documents, such as pay
stub, contract, etc., if applicable.
Note:
For Mortgage claims filed in a Chapter 13 case, attach
the Mortgage Proof of Claim (B10Attachment A) as a supportive
document.
- Print
the Notice of Electronic Filing. It's your receipt.

and 3005 regarding filing of claims by the debtor or trustee
or guarantors of co-debtors, etc.
The new Bankruptcy Rule 3002.1 requires
that when filing a Mortgage Claim in a Ch 13 case, the
Mortgage Proof of Claim Attachment (410A)
must be attached as a supportive document to the
proof of claim.
